What we collect. If you fill out the consultation request form on our booking page, we collect your first and last name, your email address, the sport you play or coach, the athlete’s age or grade, and anything you choose to write in the optional message box. If you book a call through our scheduling calendar, our scheduling provider collects the details you enter there. Our website host also records standard traffic information such as pages viewed, approximate location, device type, and referring website.
Why we collect it. We use this information to contact you about a consultation, to prepare for and deliver our services, and to understand how people find our site. We do not sell, rent, or trade your information to anyone.
Athletes under 18. We ask for an athlete’s age or grade so we know whether a parent or guardian needs to be part of the first call. If you are under 18, please have a parent or guardian submit the form on your behalf. A parent or guardian may email us at any time to see what information we hold about their child or to ask us to delete it.
Who we share it with. We use a small number of outside services to run this site and our practice: our website and form host, our appointment scheduling provider, Google Drive for storing form responses, and Google reCAPTCHA to keep spam out of our forms. Where reCAPTCHA is used, Google’s own privacy policy and terms of service also apply.
Cookies. Our website host sets cookies that are necessary to run the site and to measure basic traffic. You can block or delete cookies in your browser settings, though some parts of the site may not work as well if you do.
How long we keep it. We keep consultation requests and booking records for as long as we reasonably need them in order to serve you, and we delete them on request.
Your choices. Email us at Cschjott@psychologysupportsperformance.com to ask for a copy of the information we hold about you, to correct it, or to have it deleted. We will respond within a reasonable time.
Changes to this policy. If we change how we handle information, we will update this page and change the date at the top.
